Mission Statement

Our Mission at Qtrac is to provide software and support to LTC facilities to enable them to implement and maintain a structured Restorative Care program, thereby providing the care necessary for residents to "attain or maintain their highest practicable level of physical, mental, and psychological well-being" of which the resident is capable, as mandated by Federal and State regulations.  The computer logic supplies the vital link to ensuring quality care while maximizing reimbursement.  The program was designed to enable LTC facilities to capture Restorative/Nursing Rehabilitation care by streamlining the documentation and follow-up required.

 

Our Story

Qtrac was created by a nurse with first-hand knowledge of the everyday challenges of running a Restorative Care/Rehabilitative Care program in a long term care facility. Early in her career, she struggled with handwriting care plans and following up on residents' progress on a monthly basis for only a very small percentage of the facility who were on Restoratives. She realized that many more residents NEEDED restorative care in order to "attain or maintain their highest practical level of functioning", as required by Federal and State law. She also realized that without restorative care, many residents were vulnerable to decline.

No software was available to assist long term care facilities with the implementation and maintenance of a Restorative Care program, and thus Qtrac was created. Years in the making, it is finally available to long term care facilities at an extremely affordable cost. Qtrac is a stand-alone software program designed to complement the facilities' current MDS program. Unlike the MDS programs, however, Qtrac is a low, monthly cost and no additional computer upgrades are necessary in most cases. The program is extremely user friendly and is implemented without any disruption whatsoever to the facility. Qtrac was designed to include only the vital necessary information making maintenance of the program simple.


Qtrac is a WIN/WIN situation for the facility and its Residents. The Residents gain increased self-respect and self-esteem with increased functioning. As increased functioning occurs, the Residents require LESS nursing and C.N.A. hours. The facility can better serve its residents with additional reimbursements through restorative care. The facility saves FINES, LOST REVENUE and REPUTATION resulting from poor assessments.
